POSITION DESCRIPTION:
Ensure individuals reach their optimal level of functioning by developing individual residential habilitation plans, setting goals, and monitoring an individual's progress in reaching those goals. Provides clinical guidance to, and may supervise a limited number of direct care staff.
Primary Duties and Responsibilities
o Development of individual habilitation plans based on the individuals' valued outcomes and clinical needs of the individual, including the preparation of the residential habilitation plans as well as the development of training programs for implementation, and training of staff in the implementation of the habilitation program. o Monitoring the implementation of the habilitation plans and revised components as needed. o Completion of required habilitation documentation in line with agency and unit requirements. o Develop and participates hands-on with activities on site and in the community to foster community inclusion. o Participates in individuals' Life Plan meetings. o Participates in activities which support the program(s) and/or the unit's approach to providing optimum care for all individuals. o Oversee all aspects of special components of the program which occur regularly. o Verify all program services are provided as prescribed by team planning process.

Salary Range:
From $49202 to $62806 Annually
Minimum Qualification
Currently a permanent Habilitation Specialist I 70.1 Transfer Quals:
One year of permanent service in a direct care/paraprofessional/or professional title - Grade 12 or higher. Open Competitive:
Persons who are on the current Habilitation Specialist I Civil Service list may also apply. Consideration will be contingent upon reachability or eligibility in accordance with Civil Service Law. Simultaneous to this posting the Civil Service list for Habilitation Specialist 1 may be canvassed. Management reserves the right to select either an eligible respondent to this posting or appoint from the Civil Service list. The successful bidder to this posting is advised that it will be their responsibility to discuss, with their new supervisor, any previously approved time off. There is no guarantee the new unit will be able to honor previously approved time off. Provisional:
If appointed on provisional status, you must participate in the next Civil Service examination(s) and be a reachable candidate to obtain permanent status and maintain your position. For the Transition Examination, you must be a qualified employee of the New York State Office for People With Developmental Disabilities and have had:
Three months of permanent competitive or 55 b/55 c service in a clinical or direct care position. The list of qualifying titles is available at:

For the Open-Competitive Examination, you must have a bachelor's or higher-level degree in a human services field . For the purpose of these examinations, a degree in a human service field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, vocational counseling, social work, speech/language pathology, and therapeutic recreation (including concentrations in art, creative arts, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling/guidance, education, human development.) Examples of non-qualifying degrees include but are not limited to:
criminal justice, political science, liberal arts, and business.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
